City In Flames – Clear the Carnage Crossroads in Single Play Mode on normal difficulty.
Crash Diet – Defeat 50 Mr. Tubbs.
Crash Metal – Clear the Fright Factory in Single Player Mode on normal difficulty.
Cross-Counter – Lose all your life due to an exploding Mr. Tubbs.
Dangerous Drama – Destroy every drum in Fright Factory and clear the level.
Darren’s Dinner – Clear Fright Factory as Darren, killing 80 or more Mr. Tubbs.
Detox Attack – Defeat 50 Venoms.
Dogged Defense – Defeat 500 plague dogs.
Don’t Interfere – Defeat a Venom before it spits poison on you.
Extreme Shield – Reach Final Fear in Single Player on hard difficulty without any ally losing their life.
Faithful Firearm – Clear a stage without using a sub-weapon or letting your life reach zero.
Field Hospital – Clear Horror Hospital in Single Player Mode on hard difficulty.
Final Defeat – Get a Game Over during the Final Fear.
Fitness Rage – Defeat 500 Mr. Tubbs.
Hands Off – Clear a stage, defeating 10 plague dogs without them damagine you.
Hazardous Material – Destroy every drum in Carnage Crossroads and clear the level.
Indomitable Spirit – Clear a stage with your life in the red.
Killer Machine – Clear the Fright Factory in Single Player Mode on hard difficulty.
Max Bomber – Play as Max and deliver the final blow to the boss with a Slaughter Grenade.
Medic Meltdown – Clear a stage without picking up healing items or letting your life reach zero.
Medium Zombie – Burn 500 zombies.
Military Fatigue – Clear the Horror Hospital in Single Player Mode on normal difficulty.
Mystery Master – Play as Master and clear a stage without a main or sub-weapon, or letting your life reach zero.
Night of Kings – Clear Carnage Crossroads in Single Play Mode on hard difficulty.
O.T. Dead – Defeat 1,000 zombies.
Penicillin Panic – Defeat 500 Venoms.
Rare Zombie – Burn 50 zombies.
Skin of the Teeth – Pick up a healing item when your life is in the red.
Standard Barrier – Reach Final Fear on normal difficulty without taking any damage.
Standard Shield – Reach Final Fear in Single Player on normal difficulty without any ally losing their life.
Survivor Encounter – Play Multi Player Mode.
T.T. Dead – Defeat 10,000 zombies.
